We’re collecting experiences from people across our community – whether you were the patient yourself, supported a family member through discharge, or are completing this on behalf of someone unable to respond themselves.
Why this matters
Leaving hospital is a critical time in a patient’s healthcare journey. The transition from hospital to home can be challenging, and having the right support can prevent readmissions. Clear communication about medications and follow-up care is essential.
Your personal experiences help us identify gaps that statistics alone cannot show. Even small changes based on your feedback can make a big difference to someone’s care.
Taking part
If you or someone you know has been discharged from hospital since 1 August 2024, we invite you to complete our survey. It takes about 10-15 minutes and will remain open until 30 April 2025.
