Primary Care Networks (PCNs)

Primary care networks (PCNs) are groups of GP practices working closely together with other healthcare staff and organisations to provide more joined up care to local communities.

GP practices are grouping together and working closely in what are called primary care networks (PCNs). This means they can pool resources and give patients access to a wider team of staff such as physiotherapists, pharmacists, social prescribers and mental health nurses.  

PCNs work with other healthcare staff and organisations such as social care and voluntary services to provide more joined up care for patients and communities.  

PCNs are led by clinical directors who are medically qualified. They usually serve communities of 30,000 to 50,000 people
