Farleigh Hospice will be providing adult hospice care and bereavement support to residents throughout all of the Colne Valley from 1 February 2025.
There are six doctors’ practices in the Colne Valley area and the care they provide is co-ordinated by NHS Mid and South Essex.
Farleigh Hospice currently provides services to patients registered with three of the six GP practices in the Colne Valley and is also responsible for the provision of personal care for all local residents who are in the last stages of life.
To ensure consistent, co-ordinated care for all patients registered with a GP in the Colne Valley, NHS Mid and South Essex has decided to align all GP practices in the area. From 1 February 2025, all Colne Valley residents will receive the full provision of care that is offered by Farleigh Hospice.
Farleigh Hospice provides specialist palliative and end of life care to adults in mid Essex and specialist bereavement care for people of all ages in mid Essex. Specialist palliative care includes the management of physical symptoms, as well as supporting emotional, social, psychological and spiritual needs. Care is provided in people’s own home, in the Inpatient Unit, and in nursing and care homes.
We are very happy to be extending Farleigh Hospice’s care throughout the Colne Valley and look forward to working in partnership with our colleagues from these GP surgeries to provide specialist hospice care to patients within the community.
Thank you to NHS Mid and South Essex and St Helena Hospice for working closely with us to support a smooth transition to our care services.
