
In 2020, local GPs from the Colne Valley Primary Care Network (PCN) took a step towards combatting rising health issues in the community. They introduced the Colne Valley Low Carb Programme, managed by the NHS health and well-being coaching team. Developed by GPs at Freshwell Health Centre, the initiative has not only enabled weight loss but also contributed to significant health improvements, including diabetes remission.
The Colne Valley Low Carb Programme empowers residents to adopt a low-carb, real-food lifestyle. Equipped with educational videos, a tailor-made website full of resources, participant journals, and health coaching sessions, participants are well-supported to make life-changing decisions.
Beyond the resources, a vital aspect of this programme is the community built around it. A dedicated Facebook group and patient-led support circles provide platforms for members to share personal stories and success tales. This sense of community goes a long way in keeping the momentum going and inspiring new members to join.
The programme’s impact has been significant. Within the first 18 months, 339 patients collectively lost 1,103 kg, averaging a weight loss of 3.3 kg per person. Notably, several patients achieved remission from type 2 diabetes and pre-diabetes conditions. Many were taken off medications for diabetes and blood pressure, marking a considerable milestone for the programme. Furthermore, there were overall reductions in HbA1c, blood pressure, and cholesterol levels.
This case study exemplifies the importance of cohesive action within Primary Care Networks and the broader community. It showcases the transformative health benefits that can be achieved through innovative roles within PCNs and strong inter-practice collaborations.
