Discharge co-ordinator Heidi Webb from Essex Partnership University Trust (EPUT) has demonstrated how the Shared Care Record saved herself and Basildon Council colleagues, hours of wasted time, by accessing the patient’s details through the Cerner HIE link in ShCR, only to find out that the patient didn’t actually fall within EPUT.
The challenge
A patient arrived at Basildon Council, saying he had just been discharged by EPUT and needed a place to stay as he was homeless. Basildon Council contacted EPUT’s Discharge Co-ordinator Heidi Webb, to understand the details about his discharge and whether he was entitled to social care.
Accessing shared information
To resolve this situation, Heidi accessed the Shared Care Record to see that there was no information within mid and south Essex on this patient. However, she clicked on the Cerner (HIE) link and found that he was actually a patient of North East London Foundation Trust (NELFT) and the discharge was actually from a private hospital. This meant that the patient did not sit under Basildon Council or EPUT and could be referred back to NELFT.
Impact and outcomes
Using the Shared Care Record helped to:
- Enable accurate information in a timely manor.
- Reduced wasted time for both Basildon Council and EPUT on a patient they couldn’t care for.
- Put the patient on the right journey quickly
The difference it made
The implementation of the Shared Care Record now gives a Discharge co-ordinator an overall picture of an inpatient, which before would have taken hours from each service to obtain. By having access to safeguarding, risks and out of area data if makes the prep around an inpatient so much quicker, efficient and accurate.
