Care Navigators work in practice reception teams and are specially trained to know about the care and services available to you at your surgery and in your area.

Care navigators work in practice reception teams and are specially trained to know about the care and services available to you at your surgery and in your area. While the name of this role may differ in your practice, they’ll listen and talk to you in confidence about your health problem, to understand your needs, so they can book an appointment with the right healthcare professional or service.

What can Care navigators help with?

  • Help patients get the right care from the right healthcare professional as soon as possible.
  • Know whether self-referral is available for certain services at your practice or in your area.
  • Make appointments for new kinds of care or new services you may not be aware of.
  • Access the appropriate healthcare professional.

Hello, my name’s Sarah, I’m a Care Navigator within General Practice for the NHS.

Part of my role is when you phone or call into the surgery is to ask you what you’re here to see someone for and then I will Care Navigate you to the correct facilities that are available, whether that be through a Pharmacist, a Healthcare Assistant, a Nurse or a General Practitioner, to make sure you get the appropriate care that you deserve, both quickly and efficiently.
